* Please print
all forms on 8½ x 14 inch paper and adhere to the spaces provided when
entering information. Do not add information to the back of the forms after
printing.
Software Requirements
Minimum Requirements: Adobe Acrobat
Reader
As a minimum you must have Adobe Acrobat Reader version 4.0
to complete these forms. This can be downloaded for free from Adobe's website
at www.adobe.com . If you are
unsure about which version of Reader you have you can find out by selecting
Help --> About Reader from the menu bar.
Recommended: Adobe Acrobat (full
version)
With the full version of Adobe Acrobat you will be able to
perform all the steps outlined in the above section: "Minimum Requirements:
Adobe Acrobat Reader". In addition, using the full version of Adobe Acrobat
will allow you to save an elelctronic copy of the completed form.
General Instructions
- Completing the Forms
With Reader you can complete the
forms in two ways:
- You can save the form to your computer's hard disk,
complete the form from your computer and then print the completed form
(Recommended); or
- You can complete the forms online, and print out the
completed form.
In either case you will be unable to save a
copy of the completed form that contains your data. When completing forms with
Reader you must complete the entire form from beginning to end, and print the
form when complete. This is necessary since you cannot save your work with
Adobe Reader.
Saving a copy of the form to your pc and
launching it from your computer is preferred since the files are large and
completing them online with a 56k modem connection is very slow.
Adobe Acrobat (full version)
To save a copy of a form to your hard disk
right mouse click on the title of the desired form from the list of available
e-forms. Select "Save Target As" in the menu that appears, and save the file in
the desired location on your PC.
